I've found the easy way to improve the video modes (at least for nVidia gForce2).
My vertical refresh range is 50-180 Hz and I always got a 75Hz refresh rate when I knew it could do better.
By raising the bottom end of the range to 80Hz so it's 80-180 in XF86Config-4 I've finally gotten an 85Hz refresh rate out of the old gForce2.
Maybe this is a nVidia problem, maybe not. It would be interesting to know if this works for some other make of card.
