Mandrake 8 error messages

I have been trying to install Mandrake 8, or red hat 7.1 or Caldera 2.4 or even redmondlinux 34 and cannot get passsed stage one on any install.
the most frequent error message is Signal 11 in all distros. In Mandrake 8
I get
Fatal Error in STage one. I/o error
My system
tyan trinity s1590s 100 amd k6-2 500
voodoo 3 2000 (16 mg video memory) and usr performance pro controller based, not winmodem says on box supports linux 2.3 and higher.
I am stumped I tried usb on and off, tried cup cache internal and external on and off, PNP on and off and get the same message no matter what I do. I tried booting from the floppy from the cdrom, tried with and without a windows partition and its always the same message.
I have installed win 98 2000 and now xp on this machine without a spec of trouble but cannot get linux to even get passed stage one? I am stumped anyone have any ideas?

Must be something to do with your hardware that is a common problem for most distros.

Could you tell us a little more about the h/w especially the HD, HD controller(s) and how the drive(s) are partitioned?
